Choosing a healthcare plan can be a balancing act. Some people may prefer plans with lower premiums, while others may prefer plans with lower anticipated total costs. Another factor that might matter is whether a plan offers an associated savings account.
Note: Another factor may be network access—the Open Access PPO and Open Access HSA Plans offer access to Cigna Open Access Plus (OAP) providers, and the LocalPlus EPO Plan offers access to Cigna LocalPlus (LP) network providers. You'll save money when you stay in-network.

Unit costs above represent the average allowed charge for each service. The allowed charge is the amount of submitted charges eligible for payment for all claims according to health plan documents/contracts. Specifically, it is the amount eligible after applying pricing guidelines, but before deducting third party, copayment, coinsurance, or deductible amounts. The allowed charge represents the amount a plan will consider for adjudication for a covered service. The unit costs above include network and non-network services.

Employees who are tobacco users will have a surcharge applied to their medical plan premiums. Employees enrolling in a medical plan for the first time will be required to complete a tobacco attestation during enrollment. Lurie Children's is committed to your health and offers Tobacco Cessation Programs through Cigna.
A premium surcharge may apply if your spouse is offered medical coverage through his or her employer but you choose to cover him or her instead.
Select "No" if your spouse does not work or is not eligible for medical coverage through his or her employer. The surcharge would not apply.
Select "Yes" if your spouse is eligible for medical coverage through his/her employer and is not enrolled in his or her employer's plan. The surcharge would apply.

With a Healthcare Flexible Spending Account, you can carry over up to $610, any remaining funds are forfeited.
With a Health Savings Account (HSA), you will never forfeit any money left in your HSA — it rolls over year after year. Just remember, you can only spend HSA money that's actually been deposited into your account.
With a Healthcare Flexible Spending Account, you can only carry over up to $610 of unused money in your Healthcare FSA to the next year; you will forfeit amounts above $610. Unused Healthcare FSA dollars will be forfeited if you enroll in an HSA the following year.
Reminder: While not reflected as part of "potential rollover amount," if you enroll in the Open Access HSA, you can also contribute money to a Limited Purpose FSA to help cover eligible dental and vision expenses only.
